This commit separates AesBytesEncryptor into two separate
implememtations, allowing for a migration away from default
arrangements that used a null IV

The proxy server section pointed at RFC 7239 and then told the reader to
configure the application server for the X-Forwarded headers, conflating
the standard Forwarded header with the non-standard X-Forwarded-* set.
Describe the two kinds of headers separately, note that most proxies send
X-Forwarded-* while Spring Framework and servers such as Reactor Netty and
Jetty understand both, and state that the edge proxy has to drop or
overwrite untrusted values for both kinds rather than only one.

This commit adds support for Jackson 3 which has the following
major differences with the Jackson 2 one:
- jackson subpackage instead of jackson2
- Jackson type prefix instead of Jackson2
- JsonMapper instead of ObjectMapper
- For configuration, JsonMapper.Builder instead of ObjectMapper
since the latter is now immutable
- Remove custom support for unmodifiable collections
- Use safe default typing via a PolymorphicTypeValidator

This follows the new convention of using include-code going forward to
ensure that the documentation compiles and is tested. This also corrected
a few errors in custom params for Ballooning and PBKDF2 examples.

Documentation ids no longer need to be globally unique, so they
do not need to include the path. This makes the ids less verbose and
integrates with include-code extension better.
